Only a relatively small percentage of patients wind up requiring some distinguishing bells or whistles that may merit a higher price point. The latter should be determined by evidence based medicine, but in truth, rarely are such factors determined by clinical evidence, and in most cases bias, habit and physician-vendor relationships determine usage.
